Στην παρούσα μελέτη διερευνήσαμε μια νέα θεώρηση για το κατάλληλο ύψος εμφύτευσης της βιοπροσθετικής βαλβίδας στην δίπτυχη αορτική βαλβίδα που αποτελεί μια χωριστή οντότητα στη στένωση αορτικής που χρειάζεται διαφορετική επεμβατική προσέγγιση.
Δοκιμάσαμε μια σειρά διαφορετικών βιοπροσθετικών βαλβίδων όπως Boston Accurate, Edwards Sapien, Medtronic Corevalve και Boston Lotus και καταλήξαμε ότι το "sizing" και η εμφύτευση "ancoring" της βαλβίδας μπορεί να γίνει ασφαλώς και με καλύτερα αποτελέσματα υψηλότερα από τον χώρο εξόδου της αρ. κοιλίας.
Abstract
TAVR in bicuspid aortic valve (BAV) represents a challenging scenario. Valve sizing and positioning are the most important determinants of procedural success. In BAV the maximal prosthesis-anatomy interference occurs at the supra-annular level. Supra-annular prosthesis implantation may improve valve sealing and hemodynamic result. In this case series, we propose a new site for prosthesis implantation, guided CT scan supra-annular sizing for BAV undergoing TAVR.
Highlights
•In normal aortic valve anatomy virtual basal ring represents the gold marker of implantation and sizing; otherwise, in bicuspid aortic valve (BAV), the virtual basal ring is not the narrowest part of the aortic apparatus.
•Most of the maximal prosthesis-anatomy interference occurs at the supra-annular level. Whether supra-annular prosthesis implantation might have resulted in accurate sealing was not addressed.
•In this case series we propose a new site for prosthesis implantation, guided CT scan supra-annular sizing for BAV undergoing TAVR.
